Discipline Specific Report
This report provides a detailed overview of assessments completed by the specific discipline (OT, PT, SP). It focuses on the therapy needs identified during the assessment, offering a broad picture of how therapy can support the participant’s NDIS and therapy goals. While it highlights other needs discovered during the assessment, it doesn’t go into detail on specific hours required. This is ideal if you’re focusing on the participant’s therapy needs without the need for a comprehensive breakdown of support hours.
Plan Review report
The Plan Review report communicates the outcomes of assessments or interventions and advocates for ongoing therapy support. This report is particularly useful when a participant is approaching an NDIS plan review, as it includes clinical justification and recommendations for therapy hours and other general support needs. When multiple disciplines are involved, this report ensures a comprehensive and holistic approach, making it easier to plan for the participant’s overall well-being.
Support Plan
The Support Plan offers a concise summary of therapy goals, assessment findings, and recommendations. It’s perfect for participants who need a quick, straightforward report rather than an in-depth analysis. This plan allows therapists to efficiently analyse assessment findings and create a therapy plan, ensuring that participants receive the essential support they need without unnecessary details. The Support Plan is ideal when you need to give participants and support coordinators a clear overview of therapy findings, enabling timely decision-making and follow-up actions.
Functional Capacity Assessment (FCA) report
The FCA report provides a detailed overview of a participant’s support needs, including the recommended hours for various supports. This report is crucial for significant changes or when a comprehensive support outline is necessary, specifically outside of therapeutic-based supports. It helps to clearly define the participant’s needs in a way that supports a tailored, comprehensive approach to their NDIS plan, ensuring they receive appropriate and adequate support in key areas of their life.
Letter of Support
This letter is used to justify specific needs or requests, such as additional support hours for a specific intervention, related to the discipline completing the Letter of Support (e.g., additional hours to support participant to attend the hydro pool regularly).. While it’s not suitable for onboarding, it focuses on single, specific needs, providing clear justification without extensive assessment details. This is ideal for situations where a quick, targeted response is required to support a participant’s immediate needs.
SIL/SDA/ILO (home and living requests):
These reports support high-support needs for participants seeking specialised living arrangements. They help to justify the participants’ eligibility for different home and living support models, such as Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), or Individualised Living Options (ILO). The detailed assessments within these reports help to justify eligibility for these high-support living arrangements and ensure that the participant’s specific home and living needs are thoroughly understood and met.
Behaviour Support Plan
These reports aim to support progress towards positive change and improve the person’s quality of life, reduce the occurrence of behaviours of concern, and reduce and/or eliminate regulated restrictive practices. The report details holistic information about the person, identify the behaviours of concern and the functions of the behaviour. The plan includes person-centred, proactive and evidence informed strategies to address the underlying need of the person’s behaviour as well as response strategies to keep the person and others safe. This plan is particularly useful when identifying strategies to reduce or eliminate restrictive practices, using evidence-informed approaches to ensure the participant’s safety and well-being while promoting positive behaviour change.
